Participate in research focus groups and/or identify participants to include in focus groups in order to generate an effective future message regarding eye safety.

Provide an equipment list and specific equipment needs, including model number, manufacturer and contact information of vendor for equipment to be purchased for the school. Only the eyewear itself can be provided; not funding.

PLEASE NOTE: Submitting this agreement form does not guarantee approval for the Protective Eyewear Program. This is NOT a needs-based program. The final decision regarding participation will be based on creating a diverse group representative of Ohio’s CTE population. If your school is selected this form will be used as your commitment to fulfill all participation requirements.
